Hyperbaric Oxygen Therapy Benefits: What the Evidence Actually Shows

Hyperbaric oxygen therapy has a genuinely strong evidence base for a specific set of conditions, and a much weaker one for most of what it is marketed for. Both halves of that sentence are true, and separating them is the most useful thing anyone can do for you before you spend money on it.

Here is what the evidence supports, graded honestly.

Established: where hyperbaric oxygen is proven

These are the indications recognised by hyperbaric medicine societies and used in NHS units. The mechanism is well understood and the trial evidence is solid.

Non-healing wounds. Diabetic foot ulcers that have failed standard care heal more often with adjunctive HBOT. This is hyperbaric medicine's most widely used indication outside diving. Chronically hypoxic tissue cannot build new blood vessels or fight infection properly; raising tissue oxygen restores both.

Decompression sickness and arterial gas embolism. The original use, and still the reason coastal hyperbaric units exist. Pressure physically shrinks nitrogen bubbles while oxygen washes them out. There is no alternative treatment.

Carbon monoxide poisoning. Carbon monoxide binds haemoglobin roughly 200 times more tightly than oxygen does. Hyperbaric oxygen displaces it far faster than breathing air, and dissolved plasma oxygen keeps tissue alive while that happens.

Radiation injury. Late radiation damage to soft tissue, bone and bladder responds to HBOT by stimulating new blood vessel growth in tissue the radiation left permanently under-perfused. This is well-established practice.

Compromised grafts and flaps. Where a skin graft is failing for lack of blood supply, hyperbaric oxygen improves survival rates.

Serious soft-tissue infections. Gas gangrene and necrotising infections, where HBOT is adjunctive to surgery and antibiotics rather than a replacement for either.

Under active research: promising, not proven

This is the category most private HBOT is sold in, and it deserves a clear label rather than a blurred one.

Post-surgical and injury recovery. The reasoning is sound; healing tissue has high oxygen demand and often reduced supply; and there are positive studies in soft-tissue injury and post-operative recovery. It is not yet at the level of evidence the established list carries.

Athletic recovery and training load. Studied in sports medicine with mixed results. Some trials show faster recovery of muscle function after intense exercise; others show no difference from placebo. Enough signal to be interesting, not enough to be a promise.

Persistent fatigue. Under investigation, with some encouraging early trial data. Not an established indication, and not something to try before the common, testable causes of fatigue have been ruled out.

Cognitive function and brain blood flow. Early-stage research exists, including imaging studies showing changed cerebral perfusion after courses of HBOT. This remains preliminary work.

Not supported: what HBOT is not shown to do

Being direct about this is part of being a clinic worth trusting.

Hyperbaric oxygen therapy is not an established treatment for dementia, autism, cancer, multiple sclerosis, anti-ageing, or the broad range of chronic conditions it gets advertised for. The UK's advertising regulators have taken action against hyperbaric providers making claims of this kind, and the rules exist for a reason: people with serious conditions are the easiest to sell hope to.

If a clinic's website lists a dozen conditions with no distinction between "treats" and "is being researched for", treat that as information about the clinic.

Why the mechanism is real even where the evidence is thin

It is worth understanding why researchers keep testing HBOT, because it is not arbitrary.

Breathing oxygen at pressure raises the amount dissolved in blood plasma many times over. Plasma oxygen reaches tissue that red blood cells struggle to serve; swollen tissue, compressed tissue, tissue with damaged microvasculature. Repeated exposure also appears to trigger adaptive responses: growth factor release, new capillary formation, and shifts in inflammatory signalling.

That is a plausible basis for a wide range of effects. Plausible is not the same as proven, and the gap between them is exactly where careful clinics and careless ones part company.

Before you book hyperbaric oxygen for tiredness

If the reason you are reading this is that you are exhausted, foggy, or short of breath climbing stairs, there is a cheaper and far more likely answer to check first.

Hyperbaric oxygen increases how much oxygen your plasma can carry. It does nothing about why your blood was not delivering enough oxygen in the first place. In the UK, one of the most common reasons is iron deficiency; you need iron to make haemoglobin, and without enough of it your red blood cells cannot carry a normal oxygen load no matter how much oxygen you breathe.

Iron deficiency is common, especially in women with heavy periods, during and after pregnancy, and after surgery or blood loss. It is diagnosed with a simple ferritin blood test. It is also entirely treatable.

Treating low iron with oxygen is like topping up a leaking tank. Worth ruling out first.

How to judge an HBOT provider

Four questions that separate a clinical service from a wellness product:

  1. Do they screen you before the first session? Untreated pneumothorax, some lung conditions, recent ear surgery and certain chemotherapy drugs make HBOT unsafe. Screening is not paperwork.
  2. Is a clinician involved, or only an operator? Ask who reviews your history and who is responsible if something goes wrong.
  3. Do they grade their claims? A provider who distinguishes established from experimental is telling you they read the literature.
  4. What pressure do they run, and for how many sessions? Vague answers usually mean a low-pressure soft chamber sold at clinical prices.

The honest summary

Hyperbaric oxygen therapy is a real medical treatment with a proven role in oxygen-starved tissue, an active research frontier around recovery and fatigue, and a marketing problem at the edges. Used with realistic expectations, after the treatable causes of how you feel have been ruled out, it is a reasonable thing to try. Used as a first move for unexplained tiredness, it is an expensive way to skip a blood test.

If you are persistently exhausted, the more productive first step is finding the cause; iron deficiency, thyroid problems, sleep disorders and B12 deficiency are all common and all testable.

How many HBOT sessions before you notice a benefit?

Hyperbaric oxygen is dose-dependent, so a single session is rarely meaningful. Clinical protocols for wound healing typically run 20-40 sessions. Recovery and wellbeing courses are commonly 10-20 sessions over several weeks. Anyone promising a result from one session is overselling it.

Is HBOT worth it?

It depends entirely on why you are having it. For the established indications the evidence is strong and the benefit is clear. For recovery, training load and general wellbeing, the honest position is that research is promising but incomplete; so it is worth going in with realistic expectations and after ruling out treatable causes of how you feel.
