What Are Dangerously Low Ferritin Levels?

A ferritin level below 10 micrograms per litre is considered dangerously low and usually requires intravenous iron rather than tablets.
Ferritin is the protein your body uses to store iron. The number on your blood test tells you how much iron your tissues, muscles, and brain actually have access to. When that number falls far enough, every cell in your body starts to struggle.
Most NHS labs mark ferritin as deficient below 13 micrograms per litre (mcg/L). Clinical guidelines, including NICE CKS on iron deficiency anaemia, recognise that iron deficiency can be present even when haemoglobin remains normal, and that stores may be significantly depleted at levels well above the laboratory cut-off. A result of 8, 5, or 3 is not just "a bit low". At those levels, your bone marrow is struggling to produce enough healthy red cells, and your organs are competing for what little iron remains.
What symptoms appear when ferritin is critically low?
The symptoms of severely depleted iron stores are not subtle once you know what to look for:
- Extreme fatigue that does not improve with sleep
- Shortness of breath on minimal exertion
- Heart palpitations or a noticeably fast heartbeat
- Persistent brain fog and poor concentration
- Hair thinning or shedding in large amounts (see iron deficiency and hair loss)
- Restless legs, especially at night
- Pale skin, pale inner eyelids, or pale nail beds
- Cold hands and feet
- Frequent headaches
- Difficulty swallowing (in severe, long-standing cases)
Not everyone gets all of these. Some people with ferritin in single figures describe only crushing tiredness. Others present with palpitations that send them to A&E. The NHS guidance on iron deficiency anaemia lists breathlessness and heart palpitations as signs that need prompt assessment.
Why your haemoglobin can be normal while ferritin is dangerously low
This is the part that catches people out. Your GP may have told you your blood count is fine, yet you feel terrible. That is because haemoglobin and ferritin measure different things.
Haemoglobin measures the iron currently inside your red blood cells. Ferritin measures your stored reserves. Your body protects haemoglobin for as long as possible by raiding those stores. So you can have a normal haemoglobin and a ferritin of 6, meaning your reserves are almost gone and your body is running on fumes.

What causes ferritin to fall this low?
The most common causes in the patients we see at IronOx Clinic:
- Heavy or prolonged periods: the single most common cause in women of reproductive age (more on this at iron deficiency and heavy periods)
- Pregnancy and postpartum blood loss: iron requirements increase substantially in pregnancy, as recognised in NICE guidance on anaemia management (see also iron deficiency in pregnancy)
- Poor absorption: conditions like coeliac disease, Crohn's, or post-bariatric surgery mean dietary iron and oral supplements simply do not get through (see why some people cannot absorb iron tablets)
- Dietary insufficiency: vegans, vegetarians, and people with restricted diets are at higher risk
- Chronic blood loss: slow GI bleeding, fibroids, or frequent blood donation
- Intense exercise: endurance athletes lose iron through foot-strike haemolysis and sweat
When are iron tablets not enough?
For many people with mildly low ferritin, oral iron supplements work. But there are clear situations where tablets are unlikely to correct the problem:
- Ferritin below 15 with significant symptoms
- Proven absorption problems (coeliac, IBD, post-surgical gut)
- Intolerance to oral iron (nausea, constipation, pain)
- Ongoing blood loss that outpaces what tablets can replace
- Ferritin that has not risen after 8 to 12 weeks of consistent tablet use
